High Risk Infant Follow-Up Program (HRIF) provides home / telehealth visits by a nurse with advanced knowledge of high-risk infant care. This support opens a door of learning opportunities and development for your baby during the first two years of life.

What makes my baby high risk?
Infants born prematurely, born with a birth defect, genetic condition, or other qualifying medical conditions.

Do I have to pay for this service?
No. There is no cost to you or your insurance for this service.

What can parents expect during a visit?
Nurses will screen your baby for developmental delays, review physical assessments, provide resources and information to help promote growth and development, assist parents with services such as; special-care doctors, early intervention, immunizations, wellness care, support groups, medical transportation and other community resources.

What is the frequency of the HRIF appointments?
The nurse typically schedules appointments for the infant four times in the first year, then at 18 months and 24 months.
