The Health Access Program provides health resources to
the community through a variety of settings.Whether it be through our school-based programs
or linkages provided through KCHAIN, we are always here
to help.

The Kane
Community Health Access Integrated Network (KCHAIN) was
formed in 2004. Organized by the Kane County Health
Department, KCHAIN consists of Kane County health providers
and advocates, including the five hospitals, the federally qualified health
care centers (FQHCs),
a free clinic, two health consortia, and faith-based and
community representation. KCHAIN's primary goals are to
increase access to affordable health care for
underserved Kane County residents and to promote
preventive treatment through a primary "Medical Home."
The Pharmaceutical Access Fund (PAF) assists low-income
patients with their medication needs.The PAF is a “Last Resort” program that will
supplement (not replace) strategies that include
discounting programs and pharmaceutical company Patient
Assistance Programs.
A
county-wide Mental Health Council was developed to
achieve greater efficiencies and productivity by
leveraging resources and aid in sustaining state and
federal collaborations.
The Kane County Health
Department’s Smile for Sealants Program is designed to
support the Illinois Department of Public Health’s
mission to identify children who will benefit from the
placement of pit and fissure sealants on permanent molar
teeth.
The Kane County Health Department
offers Vision and Hearing Screening
services to preschools, daycares, and private schools.
