Kane Community Health Access Integrated Network (KCHAIN) is a partnership of health providers comprised of the Kane County hospitals, federally qualified health centers, free clinics, private practitioners, and the health department. It is funded primarily through a federal grant entitled Healthy Communities Access Program (HCAP).

All low-income insured or underinsured Kane County residents are eligible for services regardless of race, gender or age. Third party billing for Medicaid is available. No one is denied services due to an inability to pay.

KCHAIN aspires to better serve the primary and specialty care needs of this population through the assistance of case managers, benefit specialists, and existing community resources. KCHAIN will also create a pharmaceutical formulary that will be affordable and readily available to its participants.
