​Every three years, Kane Health Counts conducts a survey to learn what we need to make our county healthier. This anonymous survey will help us understand our community’s physical and mental health, quality of life, availability of services, and more. This information will be used by the hospitals and community organizations that deliver these services, and provides the data used in the development of a Community Health Improvement Plan.
