Community Health Assessment (CHA) & Community Health Improvement Plan (CHIP)

KaneHealthCounts.org is our new website to provide information about our Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). There you can explore data, build reports, review priority issues, and browse our resource library.

Periodic health assessments help us examine changes to the health of our community and help Kane County residents lead healthy, happy, flourishing lives. Combined with planning, community health assessment can be used to determine the overall health of the community and identify key health issues facing the community. It is also essential to consider all the factors that contribute to the health of a community. The definition of health now includes the quality of the community in which we live, work, and play. We still need to go to the doctor, eat healthily, and exercise; but we also must take into consideration the physical environment, social, and economic factors that influence health.

The Kane County Health Department, in collaboration with the hospitals and other community partners, is in the process of completing an updated Community Health Improvement Plan based on the newest Community Health Assessment.
