Family case management promotes healthy pregnancies and child development. We can assist with resources and referrals as well as education and support.
The Special Supplemental Nutrition Program for Women, Infants, and Children is a federal assistance program of the Food and Nutrition Service of the United States Department of Agriculture for healthcare and nutrition of low-income pregnant women, breastfeeding women, and children under the age of five.
VNA Health Care is a patient-centered, not-for-profit healthcare provider, serving over 72,000 patients a year. We offer free pregnancy testing and if you are pregnant, we’ll help you apply for Medicaid and get started with great prenatal care. Babies and children come for vaccinations, sick care and physicals (including for school). Patients with diabetes and high blood pressure get the care they need to manage symptoms and stay healthy. All ages have access to dental and vision care, mental health services( We do not complete court ordered evaluations) and low-cost prescription drugs through VNA. Home health and Hospice programs are also available. We have locations in Aurora, Elgin, Bolingbrook, Bensenville, Carol Stream, Joliet, and Romeoville. Pharmacy located on-site and also works in conjunction with Walgreens pharmacies (contact VNA pharmacy for more information).
Free home-visiting and doula services for expecting, first and second time moms of any age and income. Supports include: Doula and Home Visitor support, parent education, developmental screenings, community resource connections, advocacy, food, and caregiver supports.
