​Child and Family Connections is the point of entry into the Illinois Early Intervention System for children birth to 3 years old who have suspected or diagnosed developmental delays or disabilities.​

Eligibility: Must live in Kane or Kendall Co.  Must be BIRTH TO 3 AGE GROUP. Must have a concern with development such as cognitive, speech, motor, social/emotional,or adaptive (self help). Eligibility for the program determined by assessments/evaluation

Kane County Health Department - Coordinated Intake/iGrow

Free, family-focused home visiting programs that help parents guide their children to succeed in school and life. Provides referrals to parenting support programs in the county and some surrounding areas.​ Individual program requirements will be assessed by trained staff.

Eligibility: Please fill out iGrow Referral Form on our website to start referral process. Program takes referrals for families from pregnancy to age 3. Criteria for specific Home Visiting programs will be assessed on an individual basis.

Kane County Health Department Kane Kares Nurse Family Partnership

​Kane Kares-Nurse Family Partnership empowers first-time moms to transform their lives and create better futures for themselves and their babies. Specially trained nurses visit first time moms beginning in pregnancy and continuing through the child's second birthday.​
A nurse provides parent support, education and resources until the child's second birthday.
