Developmental Screening Provider Resources​

The following resources are intended for providers administering developmental screenings and submitting Child Find data.

Developmental Screenings to Early Intervention (EI) Referral Process

To support consistency across providers administering developmental screenings, we have added a Developmental Screening Flowchart document to serve as both a training resource and an ongoing reference guide. This tool is intended to help ensure all providers are following the same screening and referral process.

  • Child Find Screening Data Form
    Please complete one Child Find Screening Data Form per month for each zip code served and submit it to Child and Family Connections #4. Forms should be submitted by the 10th of each month.

Free ACEs Training Opportunities​​

We offer free trainings for professionals and community groups on Adverse Childhood Experiences (ACEs) and their impact on children and families. Trainings are available for school nurses, teachers, support staff, home visiting programs, and other community organizations.

These sessions provide an overview of:

  • What ACEs are
  • How ACEs can affect child and brain development
  • The connection between stress, behavior, and learning
  • Strategies to support social-emotional development and build resilience in children

Trainings are offered at no cost and are also available in Spanish.
