Kane County Housing ​Continuum of Care

The Kane County Continuum of Care’s mission is to end homelessness in Kane County.

Established in 1999, the Continuum brings together local governments and social service agencies to support individuals and families who are homeless or at risk. Partners provide outreach, assessment, shelter, permanent housing, and supportive services, working collectively to identify and address service gaps.

The Continuum secures approximately $1.5 million annually through HUD’s Continuum of Care grant process, helping fund essential programs and expand affordable housing options across Kane County.

​Coordinated Entry System (CES)

​​​The Coordinated Entry System (CES) connects people who are experiencing homelessness to housing opportunities in Kane County. The CES uses a standardized housing assessment and follows a prioritization plan to refer youth, individuals, and families to housing. The assessment is available to all eligible persons regardless of race, color, national origin, religion, sex, age, familial status, disability, actual or perceived sexual orientation, gender identify, or marital status.​

Point In Time Surveys​

The Point-in-Time (PIT) Count is a count of sheltered and unsheltered people experiencing homelessness on a single night in January. HUD requires that CoCs conduct an annual count of people experiencing homelessness who are sheltered in emergency shelter, transitional housing, and Safe Havens on a single night. CoCs also must conduct a count of unsheltered people experiencing homelessness every other year (odd numbered years). Each count is planned, coordinated, and carried out locally.
